The document outlines a formal request to change his daughter's legal name from Claudia Isabelle Barretto Baldivia (Baldivia being Dennis's real surname) to simply Claudia Isabelle Barretto officially.
Alongside the legal paperwork, Dennis added a deeply emotional text overlay to express his sadness.
