The truck suddenly swerved out of its lane and first hit a van before crashing into two motorcycles.
Two people riding one motorcycle were pulled under the truck, while the riders of the second motorcycle were thrown onto the road.
A parked motorcycle was also damaged in the crash. The people inside the van survived and were not hurt.
Police said the driver may have fallen asleep, which caused the truck to drift and hit the vehicles.
The driver is now held by the Surigao City Police. He has not released any statement about what happened.
Authorities continue to investigate the case.
