The single-vehicle crash involved a Hyundai H100 van, which was traveling along the major thoroughfare when the driver reportedly lost control, leading to a collision with the expressway’s fence.
The incident, which occurred around 8:00 a.m., has prompted a swift emergency response from local and provincial authorities.

The Municipal Disaster Risk Reduction and Management Office (MDRRMO) of La Paz was the first to receive the emergency call, immediately dispatching its response team to the scene.
The gravity of the situation required additional support, with the MDRRMO requesting assistance from the Provincial Disaster Risk Reduction and Management Office (PDRRMO), the Tarlac City Disaster Risk Reduction and Management Office (CDRRMO), and the Philippine Red Cross (PRC) Tarlac Chapter.
According to a statement from Philippine Red Cross Chairman and CEO Richard Gordon, a total of 14 individuals were involved in the crash.
Five of the passengers died at the scene, while the remaining nine suffered injuries of varying degrees.
The injured were quickly attended to by first responders from the PRC and other agencies.
Eleven of the patients were transported to the Tarlac Provincial Hospital, while three others were brought to Jecsons Medical Center for immediate medical attention.
The police have classified the incident as a "self-accident," meaning no other vehicles were involved in the crash.
Initial reports indicate that the driver of the Hyundai H100 lost control of the vehicle while traversing the expressway.
This tragic event serves as a stark reminder of the dangers of road travel and the critical importance of defensive driving, especially on high-speed expressways.
In the wake of the accident, the Philippine Red Cross-Tarlac Chapter established a Welfare Desk and Assistance & First Aid Station at the Tarlac Provincial Hospital to provide support to the victims and their families.
Manned by a dedicated team of welfare volunteers, Red Cross Youth (RCY) nurses, and Emergency Medical Services (EMS) personnel, the station is ready to offer assistance and emotional support.
The PRC also stated that its fleet of ambulances remains on standby for any further transport needs and that arrangements are in place to provide blood should it be required.
An official follow-up investigation is currently underway to determine the precise cause of the crash and to identify the victims and the injured.
